Standard Requirements for a Passport Name Change

1.  Complete Application: Complete, download & print the DS-82 application form from the government website.
   • NOTE: If your passport application processing time is 6 or more days, we will need a second copy of the application form attached to our Letter of Authorization form.

2. Complete our LOA (Letter of Authorization): The Letter of Authorization allows us to act in the capacity necessary to resolve and answer any problems that may arise, to submit your passport application and to pick up your passport (and related documents) when it has been issued by the Passport Agency.

3. Photograph: To complete a passport application for adults, you must provide two recent, clear, front-view color passport photos. These should be 2″x2″ in size and taken against a plain white or off-white background.
   1. In color photograph
   2. Printed onto matte or glossy photo quality paper
   3. 2 x 2 inches (51 x 51 mm) in size
   4. The photos should be sized so that the head measures between 1 inch and 1 3/8 inches (25 to 35 mm) from the bottom of the chin to the top of the head. Please refer to the Photo Composition Template below for additional size requirements and information.
   5. Photo must be taken within the past 6 months to reflect your current appearance
   6. Taken in front of a plain white or off-white background
   7. Taken in full-face view directly facing the camera
   8. With a neutral facial expression and both eyes open facing the camera
4. Itinerary: You must provide a copy of your round-trip ticket, reservation, or a letter from a travel agency that is addressed to you. This document needs to include the flight number, arrival and departure dates, as well as the reservation code issued by the airline or cruise line.;
5. Existing Passport:
   • Must be signed
6. Original Documents Stating Name Change: Submit one of the following documents that reflect the name you wish to have on your passport. These documents are essential for processing your name change request and will be returned along with your updated passport.
