Steps To Apply For a New Adult Passport

Eligibility for a New US Passport: US citizens 16 and older qualify for new 10-year passports. In order to submit an application:

• You are a first-time applicant who has never been issued a U.S. passport.
• Your most recent passport was issued when you were 15 or younger.
• Your last passport was issued more than 15 years ago.

1. Please Complete an Adult Passport Application Here: Finish, download & print the DS-11 application form (This form is an application for US Passport) from the US governments website.
   • Please refrain from signing the application form until a Passport Acceptance Agent directs you to do so. This form should be enclosed in the envelope provided by the Acceptance Agent.
   • NOTE: If your passport application processing time is 6 days or more, we request that you submit a second copy of the application form along with our Letter of Authorization. This duplicate should not be handed over to the passport acceptance office; instead, it must be placed inside the sealed envelope you receive from them.

2. Complete three copies of our Letter of Authorization (LOA): This document allows us to act on your behalf to resolve any issues, submit your passport application, and pick up your passport and any related documents once issued by the Passport Agency.

   • NOTE: Be sure to include an extra copy of your LOA in your packet.

3. To complete a passport application for adults, you must provide two recent, clear, front-view color passport photos. These should be 2″x2″ in size and taken against a plain white or off-white background.
   1. In color photograph
   2. Printed onto matte or glossy photo quality paper
   3. 2 x 2 inches (51 x 51 mm) in size
   4. The photos should be sized so that the head measures between 1 inch and 1 3/8 inches (25 to 35 mm) from the bottom of the chin to the top of the head. Please refer to the Photo Composition Template below for additional size requirements and information.
   5. Photo must be taken within the past 6 months to reflect your current appearance
   6. Taken in front of a plain white or off-white background
   7. Taken in full-face view directly facing the camera
   8. With a neutral facial expression and both eyes open facing the camera

   

4. Itinerary: You must provide a copy of your round-trip ticket, reservation, or a letter from a travel agency that is addressed to you. This document needs to include the flight number, arrival and departure dates, as well as the reservation code issued by the airline or cruise line.
5. Proof of US Citizenship:  You must provide an original certified copy of your U.S. birth certificate. This document should include both parents’ names, the hospital name, and must have the registrar’s raised, embossed, or multicolored seal. It should also include the registrar’s signature and the date it was filed, which must be within one year of your birth. Please note that some short (abstract) versions of birth certificates may not be acceptable for passport applications.
   • A previous valid U.S. Passport
   • Report of Birth Abroad (Form FS – 240)
   • Certification of Birth Abroad (Form DS – 1350)
   • Certificate of Citizenship or Naturalization from the USCI
6. Primary Identification: Please be able to provide one of the following documents as well.
   • A previously issued & undamaged U.S. passport
   • A Valid Driver’s License
   • A Naturalization Certificate
   • A Current Government ID (sit, state or federal). Or
   • A Current Military ID (for military applicants and dependents)
7. Payment to Government: You will need to provide a check made payable to the U.S. Department of State for $212.05, along with a $35 cash execution fee to the Acceptance Agent for your new adult passport. These fees are government charges. Your payment to Fast Passports & Visas Ohio is for the expedited processing of your passport; however, the federal government still requires its standard fees for a new adult passport.
   • NOTE: Please indicate your Date of Birth in the memo field of your check to the Department of State.
8. • Acceptance Agent: After fulfilling all the requirements for adult passport applications and once all documents have been put together you will need to have them sealed together by an Acceptance Agent.
