Passport Photograph Requirements:

- Two recent, clear, front-view, color passport photos of the applicant with a plain white or off-white background.
- Printed on matte or glossy photo-quality paper.
- Photo size: 2 x 2 inches (51 x 51 mm).
- Head size: Between 1 inch and 1 3/8 inches (25 to 35 mm) from the bottom of the chin to the top of the head. Please refer to the Photo Composition Template for further size details.
- Taken within the last 6 months to reflect your current appearance.
- The photo must be taken in full-face view, directly facing the camera, with a neutral facial expression and both eyes open.

- Itinerary: A copy of your round-trip ticket, reservation, or a letter from a travel agency in the applicant’s name, including the flight number, arrival and departure dates, and the reservation code provided by the airline or cruise company.
- Proof of U.S. Citizenship:
An original certified copy of your U.S. Birth Certificate, which must include both parents’ names, the hospital name, the registrar’s raised, embossed, or multicolored seal, the registrar’s signature, and the date the certificate was filed with the registrar’s office (within one year of your birth). Please note that some short (abstract) versions of birth certificates may not be accepted for passport purposes.

- Primary Identification: You will need to provide the following documents.

- This lost or stolen passport application form must be fully completed and signed. It is needed to prove to the U.S. State Department that you have had your Passport lost, stolen, or damaged. It must be completed in detail.
- Payment to Government:
You will need a check made payable to the U.S. Department of State for $212.05, along with a $35 cash execution fee paid to the Acceptance Agent. These are required government fees.
