If One of the parent is unable to attend :
In case of one of the parent being unable to attend the acceptance agent’s office in person, they can fill out Form DS-3053 which is a “Statement of Consent”. The other parent must present this form along with the child passport application. The DS-3053 form should be notary certified and they must submit a photocopy of their ID(front and back).
If you have the sole legal authority:
If you have the sole legal custody over the child, you need to submit a proof of your claim. You can submit the evidence in any of the following ways:

- Complete Application: Complete, download & print the DS-11 application form from the government website.
- DO NOT sign the application form until a Passport Acceptance Agent instructs you to do so. This form will go inside the envelope from the Acceptance Agent.
- NOTE: If your passport application processing time is 6 or more days, we will need a second copy of the application form attached to our Letter of Authorization form. - Proof of U.S. Citizenship: Original certified copy if U.S. Birth Certificate (must contain Both Parents names, Hospital Name, and has the registrar’s raised, embossed, or multicolored seal, registrar’s signature and the date the certificate was filed with the registrar’s office; which must be within 1 year of your birth.) - Non-Attending Parent: If both parents are not able to accompany the minor to e the Acceptance Agent office please submit the following:
- Complete, download, & print the DS – 3053 Parental consent form from the government website.
- The DS – 3053 must be accompanied by a copy of the non-attending parents’ driver’s license (BOTH FRONT & BACK)
- Photograph: two recent, clear, front view, in color, professional 2″x2″ passport photos of the applicant with a plain white or off-white background.
- Statement of Exigent / Special Family Circumstances: Use this form only if the notarized, written consent of a parent or legal guardian with custody of the minor applicant under 16 cannot be obtained.
- Your request may qualify as an exigent circumstance if there is a time-sensitive emergency and the inability of the minor to obtain a passport would jeopardize the minor’s health or welfare.
- Your request may qualify as a special family circumstance if the minor’s family situation makes it exceptionally difficult or impossible for one or both of the minor’s custodial parents/legal guardians to provide the notarized, written consent.
- Primary Identification: Please provide one of the following documents.
